Call for submissions: Innovation in Primary Care - Integration of Pharmacists

The College of Family Physicians of Canada (CFPC) has launched the Innovation in Primary Care series which aims to foster collaboration, sharing and learning among family physicians in different provinces and territories by highlighting local innovations and approaches to health care that are working well using examples of thematically connected experiences.

The Canadian Pharmacists Association (CPhA) is very pleased to announce that one of the upcoming issues will be a joint publication with CFPC.  The theme selected for this issue is “Integration of Pharmacists” which will feature exemplary instances of interaction/engagement and shared best practices between family physicians and pharmacist across Canada.

We invite you or anyone else in your local networks who might be interested in contributing to this issue, to submit a case. Links for the contributor template for case submission are below.

The themes of the previous two publications have been Effective Primary/Secondary Care Interface and Caring for Unattached and Marginalized Patients and these were very well received.

This publication is shared with CFPC’s membership (37,000+ members) through its various communication channels (website, e-Newsletters, social media) and also disseminated to its provincial chapters/territories, and Chairs and Program Directors of Family Medicine at medical schools across Canada.

The deadline for submission is September 14, 2018. Please email cases to: pemberley@pharmacists.ca
