Pharmacists/CDEs interested in developing collaborative relationships

My name is Carol Wei and I am a pharmacist/certified diabetes educator. I have been asked by the Academic Detailing group to update the current list of community pharmacists/CDEs who do insulin starts and diabetes management on the www.talksfordocs.com website

The academic detailers are a group of pharmacist who give presentations to physicians, and other allied professionals on various topics. Both members are pharmacists and their goal is to present unbiased information. Last fall their topic was insulin analogues in DM2

An extension, of their information detailing, is to provide tools and resources for the physicians. One resource list that has been included is a list of pharmacist/CDEs who would be willing and accessible, to collaborate with physicians, on insulin starts and management. The web site is www.talksfordocs.com and the list will be located in a diabetes toolkit.  Technically, the site is on the public domain so it is accessible to anyone; however, the target audience is certainly family physicians in Calgary.  A disclaimer of sorts indicating this list is not all-inclusive will be attached. e.g.  "this list may not be inclusive.". 

If you are interested in having your name on such a list, please fill out the listing authorization form and fax c/o Carol Wei at 403-948-6993 (dedicated pharmacy fax).

A response by November 30 is required, as I will be submitted the ‘updated’ list to the academic detailing group the first week in December.

The Calgary Foothills Primary Care Network (CFPCN) is a group of family physicians in North West Calgary and Cochrane collaborating with the Alberta Health Services - Calgary Zone to deliver the best possible primary care.

CFPCN brings together physicians and other primary health care providers to achieve efficiencies, assess and meet patient needs and cooperate on special programs that serve the Calgary Foothills area. The network operates in partnership with Alberta Health and Wellness, Alberta Health Services – Calgary Zone and the Alberta Medical Association.

The CFPCN Navigation Team is a new initiative that will support the family physician looking after the at-risk patient. The primary goal of the team is to connect the at-risk patient with existing community supports. The focus will initially be on:

1. To the patient discharged from hospital
2. To the patient whose physician is gathering assessment information to determine a dementia diagnosis
3. Coordination of falls prevention for primary care patients

CFPCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making. The PCN Pharmacist is an integral member of the Primary Care Network Team. The pharmacist will work collaboratively with patients, physician and other health care providers to provide clinical services as required for challenging, complex and chronic care patients.
