Patient care: Calgary Foothills Primary Care Centre

This is an innovative clinic to care for people who do not have a family physician.

We will link our patients with a family physician located in the clinic for ongoing care.

We are a multidisciplinary team of physician, nurse practitioner, diabetes educator, respiratory educator, dietitian, chronic disease management nurse, mental health worker and pharmacist.

• Any person living in north Calgary and Cochrane
